In the UK, yellow fever vaccination is only available from designated Yellow Fever vaccination centres. The Fleet Street Clinic has been a UK Yellow Fever Vaccination Centre since 1995.

Yellow Fever is a severe and potentially fatal virus infection spread by mosquitoes in Africa and Central and South America. Cases are rare in travellers, but it has a very high death rate (over 60%) if infection occurs. Yellow fever is an entirely preventable illness, and a single dose of yellow fever vaccine provides lifelong protection.

Yellow fever is a serious public health problem in the parts of the world where it occurs, with sudden outbreaks alternating with long periods of apparent inactivity. This happened on a large scale in Brazil from 2016 to 2018, and there have been ongoing outbreaks in Nigeria since 2016. Yellow fever has a high potential to spread more widely as a result of climate change, with concerns that even the United States might be at risk of its re-introduction, after an absence of a century.

Because of the scale of the problem, many countries require incoming travellers to prove that they have been vaccinated in order to prevent its spread. Failure to provide a valid certificate where required may therefore result in your being denied entry, or having to be re-vaccinated.

Travellers who are vaccinated against yellow fever are issued with an “International Certificate of Vaccination or Prophylaxis” (yellow fever vaccination certificate). Yellow fever vaccine certificates are now valid for life, even when they show a lapsed expiry date.

The certificate only becomes valid 10 days after the date of vaccination. You should therefore plan to have your yellow fever vaccine at least 10 days before your date of arrival in a country where it is a condition of entry – both to comply with formalities as well as to maximise its protection.

Formal vaccination certificate requirements are designed to protect countries from importation of yellow fever by travellers who may be incubating the infection. However, many countries have no requirements but still have a high risk of infection. If you are travelling to an area where there is a known risk of the yellow fever, you should consider getting the yellow fever vaccine for your own protection.

What is yellow fever?

Yellow fever is an acute viral haemorrhagic infection transmitted by mosquitoes.

It is a hazard to travellers and also a major public health problem, with the potential to spread rapidly within and beyond the present risk areas.

In can cause severe disease and has a high death rate despite some progress with anti-viral treatment.

It is preventable with a single dose of yellow fever vaccine, which provides immunity for life.

How do we vaccinate against yellow fever?

Vaccination against yellow fever is provided with a single jab, which is a live vaccine suitable for adults, children and infants over 9 months old.

The risk of having the vaccine increases for those aged 60 years and over, since the ability of their immune system to respond to the live vaccine virus begins to diminish with age.

It is therefore important to book a travel consultation for a risk assessment and to receive up-to-date advice about the yellow fever risk at your destination, and a review of your medical history to ensure that the vaccine can be given safely.

Who should be vaccinated?

You should be vaccinated if you will be travelling to a destination with a risk of yellow fever or if vaccination is needed as a condition of entry.

However, you should not be vaccinated if there are medical reasons why you might be more vulnerable to the vaccine’s side effects.

What other precautions should I take?

Everyone visiting countries with a yellow fever risk should take careful precautions to reduce numbers of insect bites.

Insects spread many diseases in tropical countries, not just yellow fever.

Careful precautions against insect bites can reduce the risk of infection by up to 90%.

Suitable precautions include:

  • using insect repellent at times when mosquitoes are biting,
  • covering exposed skin when outdoors
  • applying an insecticide such as permethrin to clothing, that will kill mosquitoes on contact
  • using a mosquito bed net at night
  • using an insect killer in your room.

Which countries require a yellow fever vaccination certificate for entry?

Whether you need to show a yellow fever vaccination certificate depends not just on which country you are going to, but also which country or countries you will be travelling from or have recently visited along the way.

In addition to this, if you are travelling to countries in Central and South America, or Africa, yellow fever vaccination may be recommended for your personal protection.

What is the risk of yellow fever?

The risk of contracting yellow fever depends on:

  • You travel destination
  • The current intensity of yellow fever transmission in the areas to be visited.
  • The season of travel (most cases in travellers have occurred in the late rainy season to early dry season)
  • How long you will stay
  • Activities allowing exposure to mosquitoes
  • Immunisation status

In order to prevent the international spread of yellow fever, under International Health Regulations, countries may require proof of vaccination, recorded in an International Certificate of Vaccination or Prophylaxis (ICVP).

What are the symptoms of yellow fever?

It can take up to 6 days from being bitten by an infected mosquito to becoming symptomatic.

Initial symptoms of yellow fever include fever, chills, headache, nausea, muscle aches and weakness. This can progress to high fever and jaundice (yellowing of the skin and eyes), internal bleeding and organ failure.

There is no cure for yellow fever, and the virus can be fatal in up to 60% of people who have severe symptoms.

Is there a shortage of yellow fever vaccine?

There is currently a good stock of yellow fever vaccine available.

We maintain a good stock for our patients, but we always recommend being vaccinated well in advance to allow for any difficulties, since yellow fever vaccine is very vulnerable to shortages and disruption of supply.

There is only a single manufacturer of the vaccine. All it takes to disrupt the vaccine supply chain is a sudden increase in demand if there should happen to be an outbreak anywhere in the world.

If the vaccine was administered at another travel clinic, please email our Reception at info@fleetstreetclinic.com for further assistance regarding a duplicate certificate. We will do our best to help you.

 

You need to have the vaccine at least 10 days before travelling to give it enough time to work and for your certificate to be valid.

Yellow fever vaccine contains live vaccine virus and is not recommended for everyone.

Some factors increase the risk, such as age over 60 years, reduced immunity, or disorders of the thymus gland.

It is not suitable for infants under 9 months though in special circumstances it can sometimes be used in those over 6 months.

It is therefore important for everyone to discuss their risk during a pre-travel consultation. One of our travel nurses will determine whether it is advisable for you to receive the yellow fever vaccine.

Prior to 2016, yellow fever vaccination certificates were issued with a valid of 10 years from date of vaccination. Revaccination every 10 years was the normal recommendation.

This changed on 11 July 2016, when The World Health Organization – recognising that yellow fever vaccination provides long lasting immunity – extended the period of validity of the yellow fever certificate (ICVP) from 10 years to the duration of the life of the person vaccinated. This also applies retrospectively to all people vaccinated before that date.

If you’ve been vaccinated previously against yellow fever, have no record of the vaccine details and no ability to trace them, it may be necessary to be revaccinated in order to receive a new certificate. This can be done safely, since vaccine adverse events have been associated with initial vaccination only, and not with revaccination.

Yellow fever vaccine is only available from registered yellow fever vaccination centres. You will need to visit a travel clinic, such as Fleet Street Clinic for vaccination.
